What caused this?

I am gonna get another fan but want to make sure the issue that did this is fixed first.

Bolt may have come loose, maybe something held the blade still and the fan motor kept turning? When you pull it apart you may be able to diagnose it from the inside of the fan blade to the outside of it.

Seems like a common problem across a range of different bike brands - that's probably why there are so many aftermarket fans available. Try finding a fan with metal blades.
